The Post Acute Care Service is a dedicated and passionate team of physicians and Advanced Practice Providers (APPs) (ARNPs and PAs) caring for patients discharged from UW Medical Center-Montlake; UW Medical Center -Northwest; and Harborview Medical Center.
The team provides care during the post-acute period and is designed to meet the evolving clinical needs of the patient in their home, skilled nursing facility, or other type of residence. The patient care medical complexity is wide ranging --from straight forward elective orthopedic surgery patients to patients with multiple complex medical conditions. Our goal is to improve outcomes and patient satisfaction by seamlessly transitioning patients from the care of our hospital-based teams to the Post Acute Care Service team. The APP will partner with a physician and work closely with the hospital and outpatient teams to coordinate care throughout the continuum. Currently our Post Acute Care network includes 14 skilled nursing facilities; however, we plan to add other providers over time. In addition, the service will work to implement innovative models of care by optimizing telemedicine and home-based care options.
The APP in the Post Acute Care Service works under the supervision of the Clinical Service Medical Director and the Clinical Service Manager. The Director of APPs works closely with leadership to support the clinical and professional practice of the APP team. The APP functions independently in a staff position to implement the role of expert practitioner, consultant, educator and researcher within the PAC setting. APPs possess a high degree of knowledge, skill and competence in the provision of health care. The APP performs within the scope of licensure and in accordance with clinic, hospital and university policies/standards.

Requirements: Licensure as either an Advanced Registered Nurse Practitioner (ARNP) in the State of Washington or a Physician's Assistant Certified (PA-C) in the State of Washington; must be eligible to receive hospital staff appointments for adjunct staff membership by the organized medical staff of the hospital under Medical Staff By-Laws for Hospital-based positions.
Preferred: One year of PA-C or ARNP experience desirable.
One-year Skilled Nursing and/or Home Health experience desirable.
Experience with substance abuse desirable.
